Prognostic Factors Influencing the Recurrence of Thyroid Cancer

Abstract
We undertook a retrospective study of 221 patients with histologically confirmed surgically treated thyroid carcinoma from 1971 to 1990. To determine the prognostic factors, 7 variables were chosen: patients' age, sex, tumor size, extent of
primary
cancer, lymphnode metastases, pathologic type and operation type., A univariate analysis with 10 years disease-free survival rates using Kaplan-Meier method and Log-rank statistics revelaed following significant prognostic factors: 1) Extent of
primary
cancer 2) pathologic type, and showed relative importance but not significance statistically: 1) tumor size and 2) lymph node metastases.
The risk-group definition based on age, presence of distant metastases, the size and extent of primary cancer(AMES score) by Lahey Clinic Foundation also revealed significant statistic result(p=0.0014).
A multivariate analysis using the Cox model to examine for their relationship to deseasefree survival rates revealed that the extent of primary cancer and pathologic type were also significant variables predictive of recurrence from surgically
treated
thyroid cancer.
The recurrence hazard ratios of medullary/differentiated cancer, and extra/intra thyroidal cancer were about 1.5.
